Apply for Benefits

You will be entitled to the balance of your account when you retire, become disabled or otherwise separate from an employer who is contributing to the Plan. You must request an application for benefits from the Plan's Recordkeeper.

Please contact John Hancock for assistance with applying for retirement, disability, or death benefits:

  • 24 hours a day via either the internet at mylife.jhrps.com or an automated telephone system at 800.294.3575.
  • 8 a.m. to 10 p.m. Eastern Time by calling 800.294.3575 to speak with a Participant Service Representative.

What if I am planning to retire?

To request a retirement benefit application:

  • Contact John Hancock Retirement Plan Services at 800.294.3575 between 8 a.m. to 10 p.m. Eastern Time to speak with a Participant Service Representative.

What if I become disabled?

If you become totally and permanently disabled, you can receive a disability benefit. You are considered totally and permanently disabled if you are entitled to receive a disability pension from any other qualified pension plan sponsored by the Contributing Employer or the Trustees or from any government Plan.

You must apply for a disability benefit and provide proof of your disability.
